I have no doubt, based on my research, that Lilly is dressed in a nurse's uniform. This photo could be as late at WWI, but the puffy sleeves, wider skirt and mop cap put it about 1900. After 1900 the sleeves lost all of their "mutton leg" look, the skirts narrowed and the cap was turning into a flat sided nurse's cap. That is not to say an older nurse like Lilly wouldn't be wearing dated styles, but one would think she would be in her smartest and newest uniform for a portrait.
